PLAB vs MLA Exam: Differences, Impact, and What It Means for IMGs and UK Medical Graduates

The transition from the Professional and Linguistic Assessments Board (PLAB) test to the Medical Licensing Assessment (MLA) standards marks a pivotal moment in medical licensing exams in the UK. For IMGs and UK Medical Graduates

The alignment of the PLAB exam with MLA standards signifies a unified approach to assessing medical professionals, regardless of their origin. Here’s what it means for both groups:

  • International Medical Graduates (IMGs): IMGs should prepare for a more structured and possibly more challenging PLAB exam ( For the past few years GMC has been pilot testing MLA AKT structured questions in the PLAB 1 exam, and we have noticed the exam difficulty increasing over the years. The transition to MLA standards ensures that IMGs will be assessed under the same stringent criteria as their UK counterparts, maintaining fairness and high standards in medical licensing.

  • UK Medical Graduates: For UK students, the introduction of the UKMLA replaces the final medical school exams a, standardizing the assessment process. This change ensures that all UK medical graduates meet the comprehensive competencies required to practice medicine effectively in the UK.

Exam Structure and Content

The PLAB exam, divided into PLAB 1 and PLAB 2, assesses knowledge, skills, and behaviors through the PLAB blueprint. With the transition to MLA standards, the blueprint will be replaced by the MLA content map, detailing essential knowledge and competencies for practicing in the UK. Despite this shift, the exam format and standard will not change significantly, ensuring a familiar test day experience for candidates.

  • PLAB 1 will align with the MLA Applied Knowledge Test (AKT), focusing on theoretical knowledge. ( From August 2024 onwards)

  • PLAB 2 will conform to the MLA Clinical and Professional Skills Assessment (CPSA), emphasizing practical skills. ( 17 May 2024)

As per GMC UK : The PLAB test will keep its current name. However, they plan to modernise the name of the test at a later date.

For UK medical students, the MLA will consist of two parts: the AKT and the CPSA, similar to the current PLAB structure. This ensures a consistent assessment framework for both IMGs and UK graduates.

Impact on Candidates

  • IMGs: Those eligible for the PLAB exam will remain qualified under the new MLA standards. The GMC assures that the transition to MLA will not affect the eligibility criteria, including exemptions for certain European qualifications. This transition aims to streamline the qualification process for IMGs, ensuring they meet the high standards expected of UK medical practitioners.

  • UK Medical Graduates: For UK students, the MLA will replace their medical school final exams. This change underscores the GMC's commitment to maintaining rigorous standards for medical graduates, ensuring they are well-equipped to practice medicine in the UK.

What is the format of the MLA test?

The UKMLA is a two-part assessment exam like the PLAB exam. It is called:

  • AKT (Applied Knowledge Test) 

  • CPSA (Clinical and Professional Skills Assessment). 

Both components will assess a range of knowledge and competencies mentioned in the MLA syllabus, also known as the MLA content map, which is similar to the PLAB 1 exam syllabus. Our platform has been updated to align with the MLA content map

What is the format of the CPSA (Clinical and Professional Skills Assessment) exam?

CPSA (Clinical and Professional Skills Assessment) exam will be similar to PLAB 2, which is an Objective Structured Clinical Examination (OSCE) exam and will take place at GMC’s Clinical Assessment Center in Manchester.
